Your initial appointment will include a thorough examination, review of medical history, and an evaluation of any necessary radiographs.During the consultation with one of our experienced oral and maxillofacial surgeons, a diagnosis and detailed explanation of all of your treatment options will be provided.
Occasionally, in the case of an emergency, surgery can be performed the same day (after a brief consultation and diagnosis).
For your convenience and to save time, the form that will be needed at your consultation are available online and can be sent to our office by registering online. You may also be required to complete a Medical Release. This is also available on our website if you would like to review or complete it before visiting one of our offices.
Please assist us by providing the following information at the time of your consultation:
- Any x-rays, if applicable. If your dentist or physician has taken x-rays, you may request that they forward them to our office. If additional films are necessary, they can be taken at our facility.
- A list of medications you are presently taking.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
